Transitions and Moving On
Meeting the needs of young people with learning disabilities and their families requires a flexible, adaptable approach as they move through different stages of life.
At Chiltern Centre we support this by:
- Chairing reviews and attending social care, health and education reviews as appropriate
- Participating fully in transition planning
- Helping young people and their families prepare for and experience change
- Responding flexibly to the changing needs of young people and their families, for example by changing the care we provide or the way we provide it, and always keeping channels of communication open
- Providing extra help during times of crisis
- Monitoring young people’s health status and providing information and feedback for professionals involved in a young person’s care
